Required Infomation
When placing tissue orders, the following information is required:
- name of person placing order / hospital name / phone number
- specific tissue type and size
- surgery date and time
- procedure to be performed
- surgeon name
- patient name
- purchase order number
When Ordering Osteoarticular Grafts
Osteoarticular grafts require this additional information:
- operative side (left/right)
- desired allograft dimensions
- patient's height, weight, age and gender
Patient x-rays are also helpful. If time allows, an x-ray of the best-matched graft can be sent for the surgeon's review prior to tissue delivery. A dissection description of the graft detailing dimensions also can be faxed in advance for review.
When Ordering Cardiovascular Tissue
When ordering cardiovascular tissue, it is necessary to provide the desired valve or conduit diameter and length. A dissection description of each cardiovascular tissue can be faxed to ensure the graft meets the surgeon's specifications.
